From prototype to long-standing intervention.

Transitions to Moments that Matter is currently in its prototype and refinement phase. The product itself is designed for long-term dissemination through CHYP and partner organizations.

What This Work Draws On

TMM's development is grounded in lived experience, partnership, and ongoing evaluation infrastructure.

TGNC lived experience and stakeholder knowledge informing the development and review of TMM.
Community Design Team and continued stakeholder participation.
Partnership with the California Homeless Youth Project (CHYP).
Research on stigma, youth homelessness prevention, belonging, family and institutional experiences, and access to support.
Documentary production resources and the TMM digital platform.

Q-Nife LGBTQ+ Family Resources with state-specific information and pathways to support.
Consent-governed Digital Archive with submissions reviewed before publication.
Short-form content supporting mini-series dissemination.
Dedicated TMM presence through CHYP's You Count Data Hub.
Evaluation infrastructure supporting post-viewing surveys, platform analytics, resource-use tracking, and RE-AIM reporting.
